PASRR referrals and Level II follow-up were not completed for residents with mental health diagnoses
Summary
The facility failed to ensure that Level I PASRR screenings were referred and followed up on for 3 of 5 residents reviewed for PASRR. Resident 1 was admitted with diagnoses including heart failure, depression, and anxiety. A Level I PASRR dated 05/20/2024 documented that a Level II evaluation referral was required for serious mental illness, with comments noting a history of depression and anxiety and use of an antidepressant medication. The resident’s record did not contain an updated PASRR, and the progress notes did not document a Level II evaluation. On 05/14/2026, the Social Services Director documented that they had requested an invalidation or a Level II PASRR from the PASRR coordinator and were awaiting a response. Resident 8 was admitted on hospice with diagnoses including cerebrovascular disease, dementia, and chronic kidney disease. A Level I PASRR dated 11/06/2025 documented positive indicators for serious mental illness, including mood disorder and anxiety, but also indicated that no Level II evaluation was needed because the person did not show indicators of SMI. The comments stated the resident was on hospice, nearing end of life, and that anxiety and depression were stable with current medications. The Social Services Director later stated the PASRR was incorrect and submitted a new PASRR on 05/26/2026, which documented that a Level II evaluation referral was required for SMI and noted the resident remained on hospice, had dementia, and was receiving antipsychotic, antianxiety, and antidepressant medications. Resident 4 was admitted with diagnoses including anxiety disorder, depression, and PTSD. Their PASRR dated 11/24/2025 documented known indicators of serious mental illness and an exemption from Level II evaluation because the resident was anticipated to stay less than 30 days. Progress notes showed the Level II evaluator was notified after the resident remained past 30 days and later indicated the evaluator was planning to complete the Level II evaluation, while the Social Services Director stated there was a backlog and that follow-up occurred every 30 days to six weeks.
