Failure to Provide Scheduled Bathing Assistance
Summary
The facility failed to provide bathing assistance for residents who were unable to complete ADLs independently. Review of the facility policy showed qualified nursing staff were to provide at least two full baths or showers per week, document the date and time, and record refusals with notification to the licensed nurse. The deficiency involved Residents 3, 5, 7, and 50, all of whom had care needs requiring assistance with bathing and whose bathing preferences were not consistently included in their care documentation. Resident 3 had a stroke with right-sided paralysis and speech deficits and was assessed as needing extensive assistance with bathing. The care plan did not include bathing preferences. Resident 3 stated they were not receiving showers as scheduled, said they were supposed to have showers on Mondays and Thursdays, and reported they had not been asked about bathing preferences. Bathing records showed eight scheduled showers in May 2026 with four showers and two bed baths documented, no refusals, and April 2026 records showed eight scheduled showers with only one received and several entries marked N/A without explanation. March and February 2026 had no bathing documentation available. Resident 5 had diagnoses including cerebral infarction with hemiplegia, dementia, and multiple sclerosis and required extensive assistance with bathing; the Kardex did not include bathing or grooming guidance or preferences. Resident 5 was scheduled for two showers weekly in April 2026 but received six showers rather than eight. Resident 7 had depression, low back pain, muscle wasting, atrophy, and weakness, required moderate assistance with bathing, and had a history of nightmares related to being stuck in a bathtub prior to admission; this was not included in the Kardex. Resident 7’s April 2026 records showed six showers rather than eleven and May 2026 records showed eight showers rather than thirteen, and the resident was observed with greasy, uncombed hair and body odor. Resident 50 had depression, heart failure, and muscle weakness, required moderate assistance with bathing, and had bathing preferences omitted from the Kardex; records showed only one shower 13 days after admission, and staff interviews indicated shower aides were frequently pulled to work on the floor and that missed showers and refusals were not consistently documented.
