Daily Nurse Staffing Postings Not Maintained or Updated
Summary
The facility failed to ensure daily nurse staffing information was posted and updated at the beginning of each shift to reflect staff absences, the total number of staff on each shift, and the total actual hours worked for the three months reviewed. On 07/13/2026 at 10:00 AM, the staffing information posted by the front entrance contained only a limited number of staffing sheets for selected dates in May, June, and July 2026, rather than a complete daily record. The facility also failed to maintain the posted daily nurse staffing data for the required minimum retention period, as the available postings were the only ones staff could locate. Review of the 06/22/2026 staffing posting showed three shifts with listed RN and LPN coverage and hours that did not match the facility’s Daily Staffing Assignment sheet for that same date. The posting reflected 64 hours of RN coverage and 104 hours of LPN coverage, while the assignment sheet showed 5 RNs and 8 LPNs working 12-hour shifts for a total of 60 RN hours and 96 LPN hours. During interview, the Staffing Coordinator stated the facility posted staffing the day before, placed weekend projected staffing sheets behind others so weekend staff could change them, and acknowledged that nurses sometimes did not update the posting at the beginning of their shift. The Staffing Coordinator also stated the facility sent staffing sheets to shredding after three months and did not have a system to ensure the postings were timely, accurate, or maintained for the required retention period. The DON stated the daily staff postings should have been readily available, accurate, updated at the beginning of each shift, and maintained for three years, but were not.
