Dialysis Care and Communication Deficiencies
Summary
The facility failed to provide evidence of dialysis care and services for a resident with ESRD, hemodialysis, and DM. The resident’s care plan and physician orders directed dialysis three times weekly and required monitoring of the hemodialysis site for signs and symptoms of complications every shift. However, the treatment administration record did not show monitoring of the hemodialysis site on day shift for two dates, and there was no evidence that a snack bag or early meal tray was provided when the resident went to dialysis. The resident stated she did not take a snack bag or meal to dialysis, and the dialysis communication book could not be located when requested. The dietary manager stated the kitchen prepares snack bags or trays for dialysis residents when nursing notifies dietary, and then said the resident may not be getting them. An LPN stated the resident should be receiving an early meal tray or snack bag and that it should also be documented on the MAR like other dialysis residents; when asked whether there was evidence it was provided without documentation, the LPN stated there was no evidence of it being provided. The administrator and DON were informed of the findings. The facility also failed to implement a complete dialysis communication program for another resident receiving dialysis. The resident’s orders identified dialysis days, chair time, pickup time, and transport information, and the care plan directed that a communication book be sent to dialysis and reviewed upon return. Review of the dialysis communication record did not show completed communication forms for multiple dialysis appointments. Staff interviews stated that the communication form should be completed before each dialysis trip, placed in the communication book, sent with the resident, and reviewed when the resident returned.
