Summary

The facility failed to ensure a resident’s discharge summary was completed before the resident signed it and failed to appropriately coordinate and transfer prescribed discharge medications with the resident upon discharge. The resident was admitted with diagnoses including a left femur fracture and anemia and later expressed a desire to discharge to an assisted living facility, which accepted the transfer and arranged wheelchair transportation. The record showed that Nitrofurantoin 100 mg capsules and Oxycodone were delivered to the facility and received by nursing staff before discharge, but the nurse who discharged the resident confirmed the Oxycodone was not provided to the resident and the Nitrofurantoin was received after the resident had already left. The DON acknowledged the Oxycodone had been ordered and received before discharge and stated staff were expected to clarify discharge medication orders with the physician when there was uncertainty about whether a controlled substance should accompany the resident. The nurse who completed the discharge also confirmed the discharge summary had not been completed before the resident signed it and stated the practice was not consistent with professional standards. Medical records staff confirmed the discharge summary contained blank sections despite the resident’s signature, and the DON acknowledged the discharge information should have been completed before the signature was obtained so the instructions would be complete, appropriate, and understood. The facility policy titled Discharge Summary and Plan stated that when discharge was anticipated, a discharge summary and post-discharge plan were to be developed and a copy provided to the resident and receiving facility, with a copy maintained in the medical record.
