After the incident, a physician order directed 1:1 supervision for exit-seeking behavior through 07/06/2026, with re-evaluation for continued need, but the medical record lacked evidence that the resident was re-evaluated on that date. The DON confirmed the re-evaluation was not performed and that the 1:1 supervision was not recorded as provided after the order end date because the order ended without provider re-evaluation.
