Missing Bathing and Catheter Care Documentation
Summary
The facility failed to maintain accurate and complete clinical records for a sampled resident when bathing documentation was missing and refusal documentation could not be verified. Resident #44, who was admitted with diagnoses including need for assistance with personal care, psychosis, and major depressive disorder, stated that scheduled showers on Sundays and Wednesdays were not occurring and reported going 23 days without a shower. The resident said the last shower received was on 06/21/2026 and described feeling dirty and crappy because of the lack of bathing. The resident’s documentation for May 2026 lacked entries showing a shower was provided or declined for two extended periods, and the care plan documented a need for assistance with personal care and a preference for showering two days per week with one staff assist. The facility also failed to document catheter care for residents with indwelling urinary catheters. Resident #59 had diagnoses including resistance to multiple antimicrobial drugs, UTI, and neuromuscular dysfunction of the bladder, and had an indwelling Foley catheter with a care plan requiring catheter care every shift. The resident’s task record lacked a scheduled catheter care task, and the clinical record lacked evidence that catheter care had been provided. Staff interviews indicated CNAs were responsible for catheter care and cleaning, nurses were responsible for catheter changes, and catheter care was expected every shift, but the MDS Coordinator could not locate documentation verifying that care had been completed. Resident #5 also had an indwelling Foley catheter and diagnoses including osteomyelitis of the vertebra, diabetes mellitus type 2 with neuropathy, and a stage 4 sacral pressure ulcer. A physician order directed daily and evening shift Foley site care for skin impairment management, but the task record lacked a catheter care task and the clinical record lacked evidence of catheter care. The care plan included enhanced barrier precautions related to the catheter but did not include a plan for Foley catheter care interventions and goals. Resident #18, who had multiple sclerosis and major depressive disorder, also had a Foley catheter and a current UTI. The care plan required catheter care every shift and included interventions for infection and UTI, but the task record lacked a catheter care task and the clinical record lacked evidence that catheter care had been provided. Staff interviews confirmed catheter care was expected to be documented in the EMR, but no documented evidence could be produced for these residents.
