Failure to Screen Residents for Pneumococcal Vaccine Eligibility
Summary
The facility failed to ensure 5 of 5 residents reviewed for immunizations were properly assessed for pneumococcal vaccine eligibility before vaccine decisions were made. The report states the facility did not assess immunization status, determine whether additional pneumococcal doses were indicated under current CDC recommendations, or identify contraindications or precautions before making vaccine determinations. The deficiency involved Residents #15, #5, #59, #46, and #1, and was cited as Substandard Quality of Care. Resident #15 was admitted with diagnoses including acute respiratory failure with hypoxia, type II diabetes mellitus without complications, chronic kidney disease stage 3B, and acute diastolic congestive heart failure, and later had pneumonia added as a diagnosis. The resident had a standing vaccine consent form that did not document screening for eligibility. The Nevada Immunization Record showed one dose of PCV-20 had been given before admission, and the CDC PneumoRecs VaxAdvisor indicated the resident’s pneumococcal vaccines were up to date and no additional pneumococcal vaccine was needed. However, the facility Immunization Report did not document screening for eligibility, did not document the prior PCV-20 dose, and did not document that no additional dose was necessary. Resident #5 had diagnoses including encephalopathy, type 2 diabetes mellitus with other specified complications, COPD, pulmonary embolism, and fragile X chromosome, and the representative signed a standing vaccine consent form that did not document screening for eligibility. The resident’s immunization record showed prior influenza and pneumococcal vaccines, and the CDC app indicated the resident was eligible for one dose of either PCV-20 or PCV-21 based on age and time since the last pneumococcal vaccine. Resident #59, Resident #46, and Resident #1 also had vaccine consent or immunization documentation that did not show screening for eligibility, and the facility records for these residents did not consistently document vaccine refusal dates, prior vaccine history, or whether pneumococcal vaccination was indicated or not indicated based on CDC guidance. For Resident #46, the record showed prior PCV-13 and PCV-20 doses, the CDC app indicated no additional pneumococcal vaccine was necessary, yet the facility later administered PCV-20 when the CDC recommendation indicated another dose was not needed.
