Summary

Provide for the safe, appropriate administration of IV fluids was not ensured for one resident, R171, who was admitted with diagnoses including traumatic subdural hemorrhage and unspecified dementia. On 05/18/2026, a midline with a single lumen and purple port was observed in the resident’s left upper arm with a dressing dated 05/08/2026. No IV pole or pump was observed in the room, and the medical record did not contain documented evidence that the admission nurse identified the IV line, obtained clarification orders from the physician, or transcribed care and maintenance orders into the record. During interview, the LPN confirmed the resident had a left upper arm midline that had been inserted at the hospital and stated the admitting nurse failed to identify the IV line, obtain clarification from the physician about whether to keep or remove it, and enter care and maintenance orders. The IP stated that if a resident is admitted with a midline, the nurse should obtain clarification orders from the physician and, if the line is maintained, enter orders for site assessment every shift, weekly dressing changes, and flushing. The DON also confirmed there was no documented evidence that clarification and care orders were obtained and entered into the EHR.
