Summary

The facility failed to ensure a resident received a shower as scheduled. The resident was admitted with incomplete paraplegia secondary to suspected spinal cord infarction and depression, and the quarterly MDS indicated the resident was dependent on staff for toileting hygiene and required substantial to maximal assistance for showers. A physician order dated 10/15/2025 specified a bath/shower schedule on Mondays and Thursdays from 6:00 AM to 2:00 PM. On observation, the resident appeared ungroomed and had a strong body odor consistent with infrequent bathing, and the resident stated showers were supposed to be provided on Mondays and Thursdays but that only bed baths had been received. Record review showed the resident received only 3 showers and 12 complete bed baths out of 31 scheduled bathing opportunities between 02/01/2026 and 05/19/2026, with one documented refusal and the remaining bath entries reflecting partial bed baths. On 05/21/2026, the resident reported no shower had been provided as scheduled, and the assigned CNA stated the shower was not given because no sling for the Hoyer lift could be found, so a bed bath would be provided instead. The CNA also acknowledged the shower should not have been documented as completed. Later that day, laundry staff and the EVS manager confirmed Hoyer lift slings were available in the laundry room, and the DON stated residents were expected to receive showers according to their bathing plan and for services to be accurately documented.
