Hand Hygiene and Infection Control Failures During Resident Care and Medication Pass

Benefis Senior Services - EastviewGreat Falls, Montana Survey Completed on 07-16-2026

Summary

The facility failed to follow infection prevention and control practices during resident care and medication administration. During peri-care for one resident, staff entered the room without performing hand hygiene, donned gloves and a gown, and then repeatedly touched clean and soiled items, the resident’s body, the call light, the bed remote, the resident’s hearing aid, water cup, oxygen tubing, catheter drainage bag, phone, and other equipment while wearing the same soiled gloves. Staff also handled clean briefs, chuck pads, and clothing with soiled gloves, removed gloves without hand hygiene, and continued care after touching contaminated items and surfaces. During peri-care and wound care for another resident, staff in gowns and gloves cleaned the resident’s soiled brief and peri area, then one staff member assessed and cleaned a wound on the tip of the resident’s penis while wearing soiled gloves. That staff member removed a Rover device from a pocket, used it to take pictures of the wound, set it on the bedside table, retrieved medicated ointment from the bathroom, and applied it while still wearing the same gloves. Both staff members then placed a clean brief and blanket on the resident with soiled gloves, and after removing PPE, handled the Rover device, Hoyer sling, and shorts without performing hand hygiene. During medication passes, staff failed to perform hand hygiene before and after medication preparation and between residents. One staff member scanned a resident’s chair barcode, returned to the medication cart, handled medication bottles and crushed medications, used a spoon to add pudding, and then administered the medication without sanitizing hands. The same staff member later prepared and administered medications for other residents, including removing medication cards, placing pills into cups, writing resident names on cups with a marker from a pocket, touching an internal communication device and a phone, and giving medications without proper hand hygiene. The facility’s hand hygiene policy stated that alcohol-based hand rub should be used before direct contact with patients, after removing gloves, after contact with body fluids or nonintact skin, when moving from contaminated to clean body sites, and after contact with equipment in the patient’s immediate vicinity.

EBP signage and PPE were present at the bedside, and both leaders later stated they forgot to put on gowns even though the resident was on EBP for an open wound and the facility policy required gown and glove use for wound care.

D
F0880 F880: Provide and implement an infection prevention and control program.
Short Summary

Staff failed to consistently follow EBP and hand hygiene for two residents who required high-contact care. One resident with an indwelling catheter and cognitive impairment did not have EBP followed during transfers, clothing removal, and pericare, and another resident with a suprapubic catheter and multiple pressure ulcers had staff wear PPE incorrectly, remove PPE in the room without hand hygiene, and continue care after emptying the catheter bag without changing gloves or cleaning hands. Interviews confirmed staff were expected to use gowns, gloves, and hand hygiene for these tasks.
