Behavioral Assessment and Care Planning Not Comprehensive
Summary
The facility failed to ensure that behaviors, including behaviors that were bothersome to another resident, were comprehensively assessed and that consistent, therapeutic interventions were developed and used for a resident with aphasia and cerebral palsy who was non-verbal and demonstrated physical and verbal behaviors during care and with others. The resident had documented episodes of hitting, kicking, striking out at staff during cares, and behaviors that interfered with care and placed himself and others at risk of harm. The Behavioral Symptoms CAA identified an actual behavior problem and noted multiple documented episodes, but it did not include further analysis of the incidents such as time of day, possible triggers, staff involved, or other residents involved. The record showed repeated behavior-related events, including an eMAR note indicating a behavior was observed without describing what occurred or what staff did in response. The resident was also sent to the hospital after exacerbated behaviors, including hitting another resident and two staff members during attempts to get him to bed, and his psychotropic medication was later increased with guardian consent. Staff interviews described that the resident could be difficult to redirect, sometimes struck out during cares, and was thought by some staff to target another resident. One nursing assistant stated there were no formal safety checks and that monitoring was done only when time allowed, while another stated there were no consistent staffing attempts and that the resident seemed to accept care better from certain staff members than others. The behavior care plan identified mood and behavior changes, restlessness, agitation, withdrawing, attempts to hit staff, inappropriate touching during cares, self-hitting, biting, and hollering when brought to the room. The plan included interventions such as clear communication, a fidget gadget, reminders before care, calling family, playing music, and monitoring target behaviors. However, the record lacked evidence that the interdisciplinary team had comprehensively assessed patterns or incorporated interventions staff identified as effective, including using soda pop as an incentive or assigning the same consistent staff when possible. The record also lacked evidence that the resident-to-resident interactions that were bothersome to another resident had been evaluated or addressed in the care plan.
