Housekeeping and Maintenance Deficiencies in Resident Rooms

Corbin Health And Rehabilitation CenterCorbin, Kentucky Survey Completed on 03-19-2026

Summary

The facility failed to provide housekeeping services to ensure a clean and sanitary environment for 7 of 24 sampled residents, including R3, R14, R46, R49, R71, R78, and R79. Review of the facility policy titled Protocol for Maintenance Services stated that housekeeping and maintenance were to maintain a sanitary, orderly, and comfortable interior and environment, and that any staff member could submit a work order. During observations, R3’s room had scuffed paint and marks on the wall, and R3 stated the walls had been like that since she moved in almost four years earlier and that she had asked about it a few times without anything being done. R14’s room had scuff marks where paint was missing, R46’s shower had a black substance all over the bottom, R49’s room had tiny holes and several scuffed areas with missing paint, R71’s room had paint scuffed and missing from the wall, and R78’s room had several scuffed wall areas needing paint. R79’s room also had a broken bathroom light dangling from the ceiling. The Housekeeping Manager could not be interviewed because she was on vacation. The Maintenance Director stated anyone in the facility could place a work order using little books at the nurse’s stations and said he had no outstanding projects at the time. The DON stated he was not aware of any maintenance issues and expected staff to place work orders if they saw something needing repair. Housekeeping Aide 1 stated she had been educated to notify the nurse or fill out a work order if she saw something needing repair, and that staff were instructed to notify maintenance when cleaning if they saw items needing repair. The Administrator stated it was his expectation that the facility be kept in good condition by maintenance and that residents’ walls be painted and fixed if there were scuffs or marks.
