Infection Control Breach During Wound Care
Summary
The facility failed to establish and maintain an infection prevention and control program during wound care for two residents. One resident was admitted with diagnoses including cellulitis of the left lower limb, peripheral vascular disease, and atherosclerotic heart disease, and had a wound treatment order for Dakin's solution, moistened rolled gauze, padding, and an Ace wrap to the left heel twice daily. Another resident was admitted with diagnoses including abnormal weight loss, cerebral infarction, and multiple pressure ulcers, and had a wound treatment order for cleansing with Dakin's solution, applying Santyl ointment, covering with border gauze, and changing daily and as needed. During observation of wound care for the first resident, an RN gathered supplies from the treatment cart, moistened sterile gauze with Dakin's solution, and placed it on the cart without a protective barrier. Scissors were taken from the cart drawer and used to cut a calcium alginate dressing, then returned to the drawer. The RN placed supplies on a bare, uncleaned bedside table, did not perform hand hygiene before donning gown and gloves, and removed soiled dressings that were placed on the bed. The RN did not change gloves or perform hand hygiene after removing the soiled dressings and before applying the clean dressing, and reapplied the stained elastic wrap. While wearing the same contaminated gloves, the RN touched the bed controls, bed linens, and the resident before discarding the soiled dressings and removing PPE. During observation of wound care for the second resident, the SDC changed gloves several times while moving between dirty and clean portions of the dressing change, but hand hygiene was observed only once after removing the old dressing and before donning a new pair of gloves. No additional hand hygiene was observed between the glove changes while handling the soiled dressing. Interviews with the SDC, APRN, DON, and Administrator confirmed that wound care was expected to follow aseptic technique, including hand hygiene before and after glove use, changing contaminated gloves, maintaining a clean field, and preventing cross-contamination. The facility's infection control and hand hygiene policies, as well as the nursing standards of practice reviewed, stated that staff were expected to perform hand hygiene before and after resident contact, before and after aseptic procedures, and after contact with contaminated wound dressings.
