Unsafe and Disorganized Shower Rooms and Hallway Damage

Hartland Park Health & RehabilitationLexington, Kentucky Survey Completed on 07-02-2026

Summary

The facility failed to maintain a safe, functional, sanitary, and comfortable environment in resident rooms and shower rooms. Surveyors observed holes in the walls of the [NAME] Unit hallway, including two holes on the right side of Hallway 2 above the baseboard and another hole on the left side of Hallway 1 between rooms A1 and A3. The facility also had a resident grievance-related concern documented in Resident Council Minutes, where residents identified multiple maintenance needs and the documented action was work orders. Surveyors observed the [NAME], [NAME], and Combs Unit shower rooms being used to store shared resident equipment, resident belongings, and resident care supplies in addition to serving as shower areas. The shower rooms contained mechanical lifts, wheelchairs, a shower stretcher, a bedside chair, a portable vital signs machine, transfer slings, basins, incontinence supplies, and other items stored throughout the rooms. In one shower room, a powered wheelchair remained plugged in near an electrical outlet, with a charging unit and power cord on the floor, and a hair dryer was resting on the wheelchair while plugged into another outlet. Dirty linens, tissues, wet washcloths, and residents’ personal items were also observed in the shower areas. The shower stall floors in all three shower rooms had multiple areas where the black non-skid coating was peeling, chipping, and worn, creating an inconsistent walking surface. The Maintenance Director stated electrical equipment should not be left plugged in or charging in the shower room because of water and moisture. The DON stated holes in walls should be repaired within 24 hours and expressed concern about the shower room cleanliness, equipment storage, and deteriorated floor surface. The Administrator also stated she could not explain why equipment was stored in the shower rooms and acknowledged that the storage reduced available space and detracted from the homelike environment.
