Failure to Notify Provider of Significant Blood Pressure Change

Aviva At FitzsimonsAurora, Colorado Survey Completed on 07-08-2026

Summary

The facility failed to notify the resident’s provider when there was a significant change in condition for a resident with metabolic encephalopathy, a tracheostomy, quadriplegia, and cognitive communication deficit. The resident had cognitive impairment with short- and long-term memory problems, was dependent on staff for all activities of daily living, and was observed lying in bed with tube feeding running through a gastrostomy tube. On the morning of the event, the resident’s blood pressure was documented at 69/40 mmHg, and the on-call provider was contacted with orders for free water, scheduled Midodrine, and a repeat blood pressure in one hour. The nursing note also documented that the resident was not at baseline with eye movement and was not normally responding. When the blood pressure remained low at 69/42 mmHg later that morning, the pulmonologist and physician assistant rounded on the resident, but they were not aware of the blood pressure change. The record showed continued hypotension through the morning, including readings of 75/42 mmHg and 81/42 mmHg after IV access was attempted and IV fluids were started. The physician assistant later documented follow-up for hypotension, anisocoria, and concern for stroke, stating that the resident was persistently hypotensive, did not track, was not responsive, and had abnormal pupils. The physician assistant noted that the resident had been seen by the pulmonologist that morning without pulmonary concerns, but no one had mentioned the hypotension during that round. Staff interviews indicated that abnormal vital signs and changes in condition were expected to be reported promptly to the nurse and provider, and the physician assistant stated she was not told about the hypotensive episodes until later in the morning, after the resident had already remained below baseline blood pressure for an extended period.

F0580 F580: Immediately tell the resident, the resident's doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident.
Short Summary

Failure to Notify Providers and Families of Resident Changes in Condition: Staff did not document or complete required notifications for multiple residents after new skin tears, wound care needs, refusal of ADL care, and falls/accidents. Records showed an LPN and RN assessed and dressed wounds, but provider and family notification was not documented; one resident repeatedly refused bathing without physician or RP notification, and another resident’s falls were not consistently reported to family as required.

Short Summary

The facility failed to promptly notify resident representatives when two residents fell and had changes in condition. In both cases, the provider was notified and treatment orders were received, but the records did not document representative notification at the time of the events.

F0580 F580: Immediately tell the resident, the resident's doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident.
Short Summary

Failure to notify the MD and resident representative of a significant BiPAP setting change. A resident with CHF, rheumatic heart disease with mitral stenosis, chronic respiratory failure with hypercapnia, and acute pulmonary edema had a physician order for BiPAP 28/5 with FiO2 40%. The RT changed the settings to 25/5 because the machine could not tolerate higher settings, but did not notify the MD, nursing supervisor, or representative, and did not document the change.

E
F0580 F580: Immediately tell the resident, the resident's doctor, and a family member of situations (injury/decline/room, etc.) that affect the resident.
Short Summary

A facility failed to notify the provider about missed meds for three residents and a change in condition for one resident. One resident with COPD missed 34 Duoneb treatments, another resident missed 12 hydromorphone doses and 10 pregabalin doses and later showed withdrawal symptoms, and a third resident missed 9 oxycodone doses because meds were out of supply. The record lacked evidence the provider was notified of the missed doses, and the resident with withdrawal symptoms also lacked timely provider notification of the change in condition.
