Summary

The facility failed to arrange transportation for an Ophthalmology appointment for Resident 4 after the physician ordered the appointment on 6/10/2026. Resident 4 was originally admitted to the facility and later readmitted with diagnoses that included UTI, an unspecified fracture of the first lumbar vertebra, subsequent encounter for fracture with routine healing, and dysphagia. The MDS dated 6/16/2026 indicated that Resident 4 had severely impaired cognition and was dependent on staff for toileting hygiene, showering or bathing, and lower body dressing. The Physician Order Summary dated 6/10/2026 showed an Ophthalmology appointment scheduled for 6/29/2026 at 1:30 p.m. A Nursing Progress Note dated 6/30/2026 at 12:56 p.m. indicated that Resident 4's son was informed the appointment had been canceled. During interview and record review, the DON stated that Resident 4 missed the Ophthalmology appointment because transportation was not arranged and that transportation should have been arranged in advance or within 24 hours of receipt of the physician's order. The DON stated that the failure to arrange transportation resulted in a delay in providing the services Resident 4 needed.
