Care Plans Lacked Individualized Pressure Injury Interventions
Summary
The facility failed to ensure that care plans were updated and individualized for three residents with pressure injuries when the interventions in the plans were not specific to each resident’s care needs. The report states that the baseline and comprehensive care plans were expected to include resident-specific health and safety concerns and the services, goals, objectives, and interventions needed to attain or maintain the resident’s highest practical well-being. Resident 1 was admitted with metabolic encephalopathy, aphasia, and need for assistance with personal care. Physician orders included treatment for blanchable redness to the sacral coccygeal area, a wound consult, a controlled carbohydrate diet, nutritional supplements, treatment for open discoloration to the sacral coccygeal area, a low air loss mattress, and later transfer to the hospital for evaluation and treatment. The care plan for the unstageable pressure injury to the coccyx included general interventions such as following facility policies and administering medications as ordered, but it did not include the wound care treatments, nutritional supplementation, the specialized mattress, or other resident-specific actions to prevent further skin breakdown. The DON confirmed these ordered interventions were not identified in the care plan. Resident 2 was admitted with hemiplegia and hemiparesis, a pressure ulcer, pancreatic cancer, and a gastrostomy tube. Physician orders included a wound consultation, a low air loss mattress for wound management, and treatment for a stage 2 coccyx pressure injury. The care plan for the stage 2 pressure ulcer included only general statements to follow facility policies and administer treatments as ordered, but it did not specify the ordered wound treatments, the low air loss mattress, or any individualized interventions for prevention or treatment. The ADON confirmed the care plan did not include resident-specific interventions. Resident 3 was admitted with intrahepatic bile duct carcinoma, an ileostomy, and a stage 3 sacral pressure ulcer. Physician orders included a low air loss mattress, turning and repositioning every 2 hours, offloading heels while in bed, Prostat twice daily, zinc daily, and treatment for the stage 3 coccyx pressure injury. The care plan listed only general interventions such as administering medications and treatments as ordered and educating the resident/family about skin breakdown, but it did not specify the ordered medications, treatments, repositioning, heel offloading, or low air loss mattress. The ADON confirmed the care plan did not include individualized resident-specific interventions, nutritional supplements, or the specialized mattress ordered by the physician.
