Unsafe Food Storage and Handling Practices

Harvest Crossing Post AcuteManteca, California Survey Completed on 03-12-2026

Summary

Sanitary conditions were not maintained in the kitchen and related food storage areas for 84 residents receiving meals from the facility. During observation with the Certified Dietary Manager, a black ice container was stored underneath a soap dispenser, and the manager stated there was no other place to store it until the dispenser was repositioned. The Registered Dietician later stated that placing the ice bucket under a soap dispenser posed a risk of contamination, and the facility’s ice storage policy stated that ice-storage and distribution containers can become contaminated by unsanitary manipulation, improper storage, or handling of ice. A damaged food strainer was observed in the kitchen and was being used in food preparation, and a plastic container used to store food-contact utensils was visibly dirty. Water pitchers and lids were also observed stored while still wet, with visible water droplets and pooled water beneath the drying surface, and the items were stacked closely together. The CDM and RD stated that damaged items should not be used, that utensils and equipment should be clean, and that items should be dry before storage and use; the facility’s dishwashing procedure stated that all items are air-dried with no water droplets present, and the FDA Food Code requires equipment and utensils to be clean and air dried or adequately drained. Additional observations included a dented canned food item stored in the canned goods area, a refrigerator in the nurses’ break room holding resident-labeled milk, juice cartons, cheeses, and puddings at 50 F, and a food delivery service driver in the kitchen without a hair restraint. The CDM stated dented canned goods should not be used, the IP stated the refrigerator temperature was not within the expected range, and the RD stated 50 F was not appropriate because food may spoil and cause illness. The driver stated he had not been instructed to wear a hair restraint, and the FDA Food Code requires food employees to wear hair restraints to keep hair from contacting exposed food and clean equipment.

Short Summary

Improper Thawing of Frozen Chicken: Staff thawed frozen breaded chicken in a 3-compartment sink without cold running water instead of using an approved defrosting method. A Cook said she removed the chicken from the freezer about an hour earlier, and the DM confirmed the chicken was thawed in the sink after staff forgot to remove it from refrigeration earlier. The facility's policy required thawing in refrigeration, under running water, in a microwave under specific conditions, or as part of the conventional cooking process.

F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Ice Machine and Water Filter Not Properly Cleaned: An observation found black and brown substance on the ice machine's water curtain guard and a dark brown water filter behind the machine. The dietary manager was unsure when the ice machine was last cleaned or how often the filter was changed, while the ADM believed it had been checked and cleaned within the prior week. The maintenance supervisor said he was responsible for cleaning and maintaining the machine, changed the filter monthly, visually inspected it weekly, and deep cleaned it in July, but may have missed part of the guard.
