Summary

Medication storage and labeling practices were not followed for a census of 78 when an opened carton of Readycare 2.0 nutritional supplement was found on a medication cart without an open date label. During observation, LN 2 confirmed the carton was opened and in use but not labeled, and stated the nurse who opened it should have labeled it with an open date. LN 3 also stated opened nutritional supplements should be labeled at the start of the shift, and the RD and DON both stated opened supplements were expected to have open dates and be monitored for daily changes. The medication room at one nursing station contained a staff member’s purse, clothing, cleaning chemicals, and personal air fresheners in a cabinet. The DON acknowledged the items during observation and later stated staff should not have placed personal items in the medication room, that chemicals should never have been stored there, and that the items did not meet her expectations or the facility’s process. The DON also stated the personal items created a risk for drug diversion and that the chemicals could come into contact with medications. The facility also did not have temperature logs available in either medication room, and staff confirmed the absence of temperature monitoring records during observation. LN 2 stated the temperature log should have been available because medications and supplements were stored there, and the DON confirmed the medication room temperatures were not being monitored or logged. In addition, the DON confirmed the Maintenance Director had access to the medication room and had obtained his own key, even though he was not an authorized person to access the medication rooms.
