Summary

The facility failed to keep the medication error rate below 5 percent during medication administration observations for a census of 78. Surveyors identified four medication errors out of 55 opportunities, resulting in a facility-wide medication error rate of 7.27 percent. The errors were identified during observations of medication administration for three residents: Resident 15, Resident 12, and Resident 72. Resident 15 was ordered Omega-3 Oral Capsule 1000 mg, but the nurse removed a capsule from a bottle labeled Fish Oil 1000 mg Omega-3 300 mg and administered that medication instead. During interview and record review, the nurse confirmed the medication given did not match the doctor’s order and that the resident received fish oil rather than the ordered omega-3 dose. Resident 12 was ordered [NAME]-Vite Oral Tablet, but the nurse administered a tablet from a bottle labeled [NAME]-Vite Tablets Dietary Supplement. During interview, the nurse confirmed the bottle used did not match the doctor’s order and stated the medication given may have unknown components. Resident 72 was ordered esomeprazole magnesium delayed release 40 mg via PEG tube with 50 ml of water, but the nurse mixed the medication with 5 ml of water instead. The same observation showed the nurse flushed the G-tube with 15 ml of water before medication administration and 15 ml after all medications, but did not flush between the six medications given. The nurse stated she believed flushing was only needed before and after medications. The DON stated nurses were expected to administer medications as ordered and to flush the G-tube with 15 ml in between each medication, and the facility policy required medications to be given according to the five rights and enteral tubes to be flushed before, between, and after medications.
