The facility failed to provide education, offer, and document influenza and pneumococcal vaccinations for multiple residents. During concurrent interview and record review, the Infection Preventionist Assistant confirmed that there was no evidence that the influenza vaccine was offered during the flu season in late 2025/early 2026 or that education was provided to three residents. The same review also confirmed that there was no evidence that the pneumococcal vaccine was offered or that education was provided to three residents, including two residents who were affected by both vaccine omissions and one additional resident affected by the pneumococcal vaccine omission. The facility’s undated Influenza Vaccine policy stated that all residents without medical contraindications are to be offered the influenza vaccine annually, that residents or their legal representatives are to receive information and education about the benefits and potential side effects before vaccination, and that education and refusals are to be documented in the medical record. The Pneumococcal Vaccine policy stated that residents are to be assessed for eligibility prior to or upon admission, offered the vaccine series when indicated within 30 days of admission unless medically contraindicated or already completed, and provided education about benefits and side effects before receiving the vaccine, with documentation in the medical record. During interviews, the IPA stated that influenza and pneumococcal vaccines were expected to be offered upon admission and that residents should receive education to make informed decisions. The Admissions Coordinator stated that immunizations were not offered as part of the initial admission packet. The DON stated that residents should receive influenza and pneumococcal vaccinations to help prevent disease and that education should be documented in the EHR or medical chart. The report identified four sampled residents who did not have evidence of the required vaccine offering and/or education documentation.
