Summary

The facility failed to implement appropriate infection prevention and control practices for 5 of 28 sampled residents. During a concurrent observation and interview, a CNA delivered a meal tray to one resident, assisted with meal set-up, exited the shared room without performing hand hygiene, then opened the meal cart, picked up another resident’s meal tray, returned to the room, and assisted that resident with meal set-up without performing hand hygiene between residents or after leaving the room. The CNA stated she did not perform hand hygiene between the tray deliveries or after exiting the room. The DON stated staff were expected to perform hand hygiene between delivering meal trays to different residents and before entering and after exiting residents’ rooms. The facility policy stated hand hygiene is the primary means to prevent the spread of healthcare-associated infections and is indicated after touching the resident’s environment. Multiple urinals used by three residents were observed not to be labeled with identifying information, and some were dirty with urine still in them. A nurse confirmed one resident’s urinal was dirty and unlabeled and stated it should have been labeled; she also stated urinals should have been emptied right away and that unlabeled urinals created an infection control risk because they could be switched or mixed up. Another nurse confirmed a second resident’s two urinals were dirty and unlabeled, and a CNA confirmed a third resident’s urinal was not labeled and should have been. The DSD stated urinals should have been cleaned or replaced if too dirty to clean and labeled with the resident’s initials or room number. The IPA and DON both stated unlabeled urinals could be mixed up, and the IPA stated residents could get sick if urinals were mixed up and potentially develop UTIs.
