Smoking Supervision and Paraphernalia Control Deficiencies
Summary
The facility failed to ensure an environment free of accident hazards and adequate supervision related to smoking for multiple residents. The deficiency involved residents smoking without supervision, residents keeping lighters and cigarettes in their possession when the facility records indicated those items were to be secured by staff, and missing or incomplete smoking assessments and care plans for some residents. The report identified Residents 2, 27, 30, 34, 52, 65, 71, 81, and 90 as affected. Resident 2 had diagnoses including respiratory disorders, a mandible fracture, quadriplegia at the C1-C4 level, psychosis, cognitive communication deficit, osteoarthritis of the hand, and schizophrenia. His BIMS score was 7, indicating severe cognitive impairment. The DON reviewed his smoking data collection and assessment dated 1/5/25 and confirmed that quarterly smoking reassessments had not been completed after that date, even though reassessments were needed to determine whether he could continue smoking safely. Resident 27 had COPD, transient cerebral infarction, lack of coordination, hemiplegia and hemiparesis following cerebral infarction, bipolar disorder, paranoid schizophrenia, schizoaffective disorder, and auditory and visual hallucinations. Although his BIMS score was 15, he told the surveyor that he kept his own lighter and smoked outside independently without staff supervision. The DON reviewed his smoking assessment and acknowledged that the resident had physical limitations and mental health conditions, along with antipsychotic medication that could cause sleepiness, dizziness, and decreased alertness, yet he was smoking without supervision and keeping smoking paraphernalia in his possession. Resident 30, Resident 65, Resident 34, Resident 52, Resident 71, Resident 81, and Resident 90 were also observed or reviewed as smoking without the level of supervision or paraphernalia control reflected in their records. Resident 30 and Resident 65 did not have updated quarterly smoking reassessments, and both were found smoking without supervision while keeping lighters or cigarettes with them. Resident 34 smoked without supervision and did not have a smoking care plan. Resident 52 smoked without supervision, kept smoking paraphernalia in her possession, and did not have a smoking care plan. Resident 71 smoked without supervision. Resident 81 was observed smoking alone in the smoking area while keeping his own cigarettes and lighter. Resident 90 did not have an initial smoking assessment completed, did not have a smoking care plan, smoked without supervision, and had a lighter and cigarettes left visible and accessible in his room. The DON acknowledged in multiple instances that the residents’ physical limitations, diagnoses, and medications limited their ability to smoke safely without staff supervision, and the facility policy required close supervision for residents who were physically or mentally incapacitated and staff control of smoking paraphernalia for residents unable to keep it safely.
