Failure to Follow Enhanced Barrier Precautions During Resident Care

Summary

The facility failed to establish and maintain an infection prevention and control program when staff did not follow enhanced barrier precautions for one resident who had a catheter and was on EBP. The resident’s record showed diagnoses including acute kidney failure, difficulty walking, overactive bladder, and heart failure. The physician order summary indicated the resident was on EBP and standard precautions related to the catheter, and the care plan directed staff to don gown and gloves for high-contact care activities such as bathing, transfers, hygiene, linen changes, toileting, and device care. During observation, the resident had an EBP sign outside the room stating that staff must wear gloves and a gown for high-contact care, including transferring, changing linens, providing hygiene, changing briefs or assisting with toileting, and device care. Staff observed providing perineal care, changing linens, emptying the catheter bag, dressing the resident, and transferring the resident from bed to wheelchair without disposable gowns. In interview, the Staffing Coordinator, CNA B, and CNA C stated they realized they had not put on gowns and acknowledged that gowns and gloves should be worn for high-contact care to prevent spread of infection. The DON stated the staff were nervous and forgot to put on the gown, and the resident stated nursing staff usually did not wear gowns.
