Summary

The facility failed to ensure the lunch menu met nutritional needs in accordance with established national guidelines when [NAME] F did not follow the recipe for pureeing rice pilaf. During an observation on 06/02/26 at 12:26 PM, [NAME] F was seen scooping rice pilaf into a canister to puree it and adding water, with no measurements used. [NAME] F stated he did not have a recipe for the pureed rice pilaf, said he had used to have one but did not know what the Dietary Manager did with it, and said he had been pureeing food for a while so he knew how to do it without a recipe or measurements of the additive. [NAME] C stated water had to be used because the rice pilaf was too dry to puree and would not thin out otherwise. During interviews, the Dietary Manager said she was unable to locate the pureed rice pilaf recipe, while the Dietician stated there was a recipe and that water should not be used as a diluent for pureed rice pilaf or any other pureed food. The Dietician said not following the recipe and using water could diminish the nutritive value of the food and cause weight loss, and the Dietary Manager said she was responsible for overseeing that cooks used the proper recipe and ingredients. The Dietary Manager later stated [NAME] F was supposed to follow the recipe when pureeing food, that water was never used because it had no nutritional value, and that adding too much thickener or using water could alter the consistency, flavor, and nutritional value of the food.
