Summary

Drugs and biologicals were not stored in locked compartments with access limited to authorized personnel for 2 residents reviewed. Resident #12, a female with schizoaffective disorder, bipolar type and a quarterly MDS showing a BIMS score of 10 indicating moderate cognitive impairment, was observed lying in bed on 06/01/26 with a bottle of wound cleanser sitting on the dresser at the end of her bed. The facility’s ADON stated the wound cleanser should not have been on the dresser and should have been secured in the wound care cart because a resident could potentially get a hold of it and use it for unintended purposes. Resident #36, a female with major depressive disorder and vascular dementia, mild, with an annual MDS showing a BIMS score of 03 indicating severe cognitive impairment, was observed multiple times with a bottle of stomach relief medication in the refrigerator in her room. Her physician’s orders printed on 06/01/26 did not contain an order for the medication. The ADON and DON stated the medication should not have been in the resident’s refrigerator, that anyone had access to the refrigerator, and that someone could get ahold of it; the DON also stated housekeeping was responsible for checking the refrigerator daily.
