Summary

The facility failed to store, prepare, distribute, and serve food in accordance with professional standards in its only kitchen. During the initial tour on 06/01/2026, surveyors observed the food mixer with four clumps of dried blackish/grey substance around the sides of the [NAME] head, the toaster oven with a thick, sticky layer of black, brown, and yellow crusty crumbs, and plates and trays stacked on a kitchen cart with water noted between them. The ice machine also had a thin line of runny orange liquid substance on the interior portion of the top of the cooler with a slow drip into the fresh ice cubes. The Dietary Manager stated the mixer should be cleaned after each use, including the head portion, because of splashing while in use, and said the plates and trays should remain unstacked to allow air drying to prevent bacteria growth. She also stated the kitchen did not allow enough space for proper air drying. During a later interview, she said she was not sure how to clean the toaster and would have to figure out how to remove the rollers to remove all the layers of crumbs, and that she had not checked the ice machine within the last month or so because she thought services were contracted out to the ice machine supplier. She said she monitored kitchen cleanliness by walk-throughs but did not have documentation of the checkoffs. The Administrator stated she expected sanitary food practices to be followed and said she was ultimately responsible for oversight of the Dietary Manager.
