Summary

The facility failed to ensure a resident with severe protein-calorie malnutrition, dementia, and a BIMS score of 00 received the required supervision and assistance with eating during breakfast service. The resident’s care plan and MDS reflected that she required supervision or touching assistance with eating, and that staff were to provide supervision assistance by one staff member to help her eat. Her record also showed a history of significant weight gain over the prior 90 days and that she was on a pureed diet with supplements and snacks ordered. During observation in the secure unit dining room, the resident received her breakfast tray while a CNA set up the tray and then left to pass out other trays. The resident sat staring at the meal tray and drank juice, but did not attempt to pick up utensils or eat. From 7:30 a.m. until 8:23 a.m., staff did not cue her to eat or assist her with feeding. An ADON entered the dining room and spoke with residents about coffee, but did not cue the resident to eat or offer feeding assistance. At 8:23 a.m., an LVN asked whether the resident wanted her food warmed, left the dining room, returned with another tray, and then began assisting her with feeding. In interviews, the CNA stated the resident usually needed cueing to eat and that all staff were responsible for cueing residents or assisting with feeding as needed, but she had not noticed the resident was not eating because she was feeding another resident. The LVN stated he did not cue the resident or get another staff member to assist while he was feeding another resident, and the ADON stated the resident required cueing and sometimes feeding assistance. The facility’s policy stated that residents who require dining assistance will not have trays delivered until a staff member is available to assist with dining.
