Summary

The facility failed to assist a resident in obtaining routine and emergency dental services, including arranging transportation to and from the dental appointment. Resident #2, a cognitively intact male with diagnoses including dementia, type 2 diabetes mellitus, and cellulitis and abscess of the mouth, had ongoing mouth sores, drooling, and difficulty speaking. His record showed an appointment with an oral maxillofacial surgeon was scheduled, but his care plan did not include a plan for cellulitis and abscess of the mouth, mouth sores, mouth pain, or dental follow-up. During observation, he stated he had missed a recent dental appointment because the person supposed to take him did not show up and that he did not know when he would go, though he wanted to attend. Interviews showed the facility transporter was on leave and transportation had been informally assigned to a CNA, who stated she was unable to come to work on time on the day of the appointment because of personal family issues and lack of a babysitter. The CNA said she notified the ADON and Administrator the same morning and they said the appointment would be rescheduled. The ADON stated the facility had used the ADON and a CNA to provide transportation in the transporter’s absence, that the resident did not meet medical necessity for a transport company, and that the facility van was used for appointments. The Administrator stated transportation coordination normally fell to the DON, with herself as backup, and that on the day of the appointment neither she nor the ADON could go. The facility policy stated it was responsible to arrange or provide transport as needed and to schedule transportation to meet appointments.
