The facility failed to ensure Resident #1’s out-of-hospital DNR was accurately reflected in the medical record and care plan. Resident #1 was a [AGE]-year-old female admitted with diagnoses including dementia with behavioral disturbance, type 2 diabetes mellitus, and malignant neoplasm of the left kidney. Her quarterly MDS showed a BIMS score of 2, indicating severe cognitive impairment. A signed OOH DNR was found in a three-ring binder at the nurses station, but it was not uploaded into the resident’s medical record. The electronic dashboard listed the resident as full code, and the physician order summary also showed a full code order. The care plan dated [DATE] stated the resident desired full code status. During interview, the resident’s family stated they did not want CPR for Resident #1 and had signed an OOH DNR to honor that request. The family member said the ADON helped coordinate obtaining and signing the OOH DNR. The ADON stated the OOH DNR was kept at the nurses station in a binder and that medical records was responsible for uploading it into the record, but the facility did not have anyone staffed in medical records since February 2026 and the Administrator had been doing the uploading. The ADON also stated the facility did not have a social worker or DON, and that Resident #1’s physician orders, dashboard, and care plan did not accurately reflect the family’s desire for DNR status. The facility policy stated copies of advance directives are to be maintained in the medical record and the attending physician is to be notified so appropriate orders can be documented in the medical record and plan of care.

For one resident with diagnoses including Alzheimer's disease and other medically complex conditions, the record showed severely impaired cognition and inability to make decisions. A family member reported that the resident had a decline in health about a week before hospitalization, with severe UTI and stool impaction. The hospital record documented that the resident arrived from the nursing home via EMS with confusion and altered mental status that had worsened since several days earlier, and the ED physician suspected sepsis. The resident was febrile, tachycardic, had elevated white blood cell count and creatinine, and was diagnosed with proctitis, cystitis, sepsis, NSTEMI, and acute kidney injury. The urine culture was positive for gram-negative rods/Pseudomonas aeruginosa. For another resident with diagnoses including diabetes mellitus, a history of UTIs, CHF, Alzheimer's disease, and dementia, the progress notes showed family concerns about abnormal behavior, confusion, agitation, shortness of breath, congestion, lethargy, and shallow breathing. The resident's DPOA reported repeatedly expressing concern that something was wrong and requesting hospital evaluation, but the facility did not immediately send the resident out and instead contacted the physician first. The change-in-condition documentation noted altered mental status and respiratory symptoms, and the physician initially recommended a nebulizer treatment before later directing transfer to the ED. The resident was admitted to the hospital with bilateral pneumonia, acute hypoxic respiratory failure, likely demand ischemia, chronic diastolic CHF, fluid overload, severe anemia, and human metapneumovirus infection.
