Summary

The facility failed to develop and implement a comprehensive person-centered care plan for Resident #18 that included measurable objectives and timeframes for her medical, nursing, mental, and psychosocial needs. Resident #18 was a female resident with diagnoses including Alzheimer's Disease, type 2 diabetes mellitus, and anxiety disorder, and her quarterly MDS showed a BIMS score of 3, indicating severe cognitive impairment. Her record showed repeated behaviors of going into other residents' rooms, including male residents' rooms, and removing clothing. Progress notes documented that she was redirected when entering other residents' rooms and taking off her clothes on multiple occasions, including removing linens and pillows from rooms, entering male rooms and starting to remove clothes, continuing to remove clothing, and walking in and out of other residents' rooms while taking off her clothes. The care plan last captured on 09/04/2025 did not include these behaviors. Monthly psychological assessments from March 2025 through August 2025 did not note concerns related to her wellbeing. During interviews, staff stated that Resident #18 wandered room to room, stripped beds, and undressed herself, and that they redirected her by offering activities or snacks and re-dressing her as needed. The MDS staff member stated she was responsible for completing care plans and was aware of the resident's behaviors of taking off her clothes and going to male residents' rooms, and the Administrator stated these behaviors should have been in the care plan because care plans provide staff with a snapshot of how to care for residents and communicate resident care needs.
