Summary

The facility failed to ensure Resident #40 had a working call light button attached to the call light system and accessible from the resident’s bedside. Resident #40 was an [AGE] year-old male admitted on [DATE] with diagnoses including vascular dementia, encounter for palliative care, and chronic kidney disease. His MDS assessment reflected a BIMS score of 3, indicating severe cognitive impairment, and he was dependent with all ADLs and always incontinent of bowel and bladder. His care plan noted impaired thought processes related to dementia. During observations on [DATE] at 10:05 AM and again at 12:45 PM, Resident #40 was lying in bed and did not have a call light button attached to the call light system. During an interview on [DATE] at 3:19 PM, the Maintenance Supervisor stated he fixed the call light in the resident’s room that morning and explained that before replacing the plate there had only been one call light cord in the room, which the roommate had. He also stated there was another call light connection behind the television because staff had moved the resident’s bed. During interviews, LVN E, the DON, and the Administrator each stated they expected all residents to have a call light accessible and that staff were responsible for ensuring it was available and within reach. The facility policy stated call lights should be available at each resident’s bedside, toilet, and bathing facility and accessible while the resident is in bed.
