Inaccurate MDS Coding for Bed Rails, Insulin, and Discharge Status

Summary

The facility failed to ensure resident assessments accurately reflected resident status for 3 of 21 residents reviewed. For one resident with diagnoses including Alzheimer’s disease, dementia, rheumatoid arthritis, and chronic pain, the quarterly MDS dated 3/22/26 coded a bed rail/restraint in Section P even though the resident did not have bed rails. During interview and observation, the resident was in her wheelchair, stated she was treated well, did most things herself, and said she had not been restrained in any way. The MDS nurse later stated the resident had never had bed rails and should not have been marked for bed rails/restraints. For another resident with diagnoses including hemiplegia/hemiparesis following cerebral infarction, seizures, and type 2 diabetes mellitus with polyneuropathy, the quarterly MDS dated 4/8/26 coded injections 7 of 7 days and insulin 7 of 7 days in Section N. However, the physician’s orders showed the resident did not take insulin and instead had orders for weekly blood glucose checks and metformin twice daily. During interview, the resident said she had not ever taken insulin, and an LVN stated the resident had never taken insulin as far as she knew. The MDS nurse acknowledged the coding was incorrect and said it was a mistake. For a third resident with diagnoses including acute on chronic systolic CHF, ventricular tachycardia, and an implanted cardiac defibrillator, the MDS Nursing Home Discharge dated 5/20/26 indicated discharge return not anticipated and unplanned discharge to a short-term general hospital. The record also showed the resident entered the facility again on 5/21/26 from the hospital. During interview, the MDS nurse stated the resident should not have been marked discharged return not anticipated because he had gone to the ER and was coming back, and she said it must have been a technical error. The DON and ADM both acknowledged that the inaccurate MDS entries created an inaccurate picture of the residents’ status.
