The facility failed to provide pharmaceutical services to meet the needs of one resident when a medication administration error occurred involving Eliquis. The resident was admitted with diagnoses including atrial fibrillation, hypertension, and need for assistance with personal care, and his admission assessment showed a BIMS score of 8, indicating moderately impaired cognition. He stated Med-Aide D administered Eliquis without reviewing the MAR in the computer system on the medication cart. Med-Aide D stated she did not review the MAR before giving the medication and acknowledged she did not follow the expected medication administration steps, including reviewing the MAR and comparing it with the medication being administered. The resident’s procedure was postponed after the medication error, and the report states the surgery originally planned for 06/26/2026 was delayed until 06/29/2026.
