The facility failed to develop and implement a comprehensive person-centered care plan for Resident #5 that reflected her current activity preferences and included measurable objectives and timeframes to meet her psychosocial needs. Resident #5 was an [AGE]-year-old female admitted with diagnoses including cognitive communication deficit, panic disorder, and age-related cognitive decline. Her admission MDS, dated 05/13/2026, showed a BIMS score of 15, indicating intact cognition, and her activity preferences identified as very important included having books, newspapers, and magazines to read, being around animals, keeping up with the news, and going outside for fresh air when the weather was good. Record review showed the comprehensive care plan, dated 5/25/2026 and revised on 06/02/2026, was not updated to reflect that Resident #5 needed in-room activities. The list of residents on the in-room activity program reflected she was to receive in-room activities, but during observation and interview on 06/30/2026, she was lying in bed watching television and stated she preferred someone to come to her room and do activities rather than attend group activities. She said she had not received activities in her room and would enjoy doing an activity with someone outside just the two of them or playing a game in her room. The Activity Director stated Resident #5 was on the in-room activity program but her care plan was not revised to meet her current activity preferences. The MDS Coordinator stated the activity care plan was expected to be revised when preferences changed and that in-room activities were not on Resident #5's current care plan. The Administrator stated care plans were expected to be revised when resident preferences changed and that resident information needed to provide appropriate care should be documented on the care plan.
