Summary

The facility failed to ensure that one sampled resident was assessed for safe self-administration of medication before self-administering Testosterone Transdermal Gel 1.62%. During a medication pass observation, an LVN placed one pump of the gel into the resident’s outstretched, ungloved right hand, and the resident applied the gel to his left forearm. The resident did not wash his hands after applying the medication. Immediately afterward, the LVN stated the resident should have had an order to self-administer the testosterone gel and should have washed his hands after application, and verified that the resident should not have been allowed to self-administer without an assessment and order. Record review showed the resident was admitted with the capacity to understand and make medical decisions, and had a physician’s order for Testosterone Transdermal Gel 1.62% to be applied once daily for hormone replacement. However, the record did not show a physician’s order for self-administration of the medication, documentation of an assessment for self-administration, or a care plan addressing self-administration. The DON later confirmed these findings.
