Broken Freezer Not Maintained in Safe Operating Condition

Dove Healthcare - SuperiorSuperior, Wisconsin Survey Completed on 05-13-2026

Summary

The facility did not maintain mechanical and/or electrical equipment in safe operating condition when surveyors observed a freezer in the kitchen storage area with orange duct tape on the lid, the lid sagging, and two heavy syrup bottles being used to hold it shut. The freezer had ice build-up along the sides at the entrance of the lid around a cracked seal, and the freezer was being used to store frozen food for all 58 residents in the facility. Surveyors also observed frozen food inside the unit, including a gallon Ziploc bag dated 05/06 with two sections of roast that appeared freezer burnt and had ice build-up inside the bag, as well as fried rice with ice build-up inside the package indicating freezer burn. During interview, kitchen staff reported the freezer had been broken for over 6 months and that the issue had been reported to the Director of Nutritional Services and then to Maintenance. Maintenance staff stated they were unsure whether staff had reached out about the broken freezer and said they could order the needed part. The Director of Nutritional Services later reported the freezer had been broken for months, that kitchen staff had to chip away at the ice build-up, and that Maintenance had been informed of the broken freezer 6 months earlier.
