Failure to Provide Behavioral Health Services
Summary
The facility did not ensure that residents received necessary behavioral health care and services to maintain the highest practicable mental and psychosocial well-being in accordance with their comprehensive assessments and plans of care. The deficiency involved 3 of 5 sampled residents reviewed for mood concerns: R2, R24, and R30. The report also states that on 5/26/2026, the Nursing Home Administrator told the surveyor that the facility did not have a policy and procedure for initiating behavioral health care and services. R2 was admitted with diagnoses including PTSD, major depressive disorder, paraplegia, end stage renal disease, morbid obesity, type 2 diabetes mellitus, and COPD. R2’s MDS dated 2/2/26 showed a BIMS score of 14 and no depressive symptoms or behaviors. During interview, R2 stated that R2 had been offered psychology services but wanted psychotherapy in person rather than by zoom. The Social Services Coordinator confirmed that the facility only offered zoom psychology services, made no attempt to provide in-person services, and that R2 had been trying to receive talk therapy at the facility but would not do zoom psychotherapy. R24 was admitted with diagnoses including type 2 diabetes mellitus, essential hypertension, anxiety disorder, and depression. R24’s MDS showed a BIMS score of 15 and a PHQ-9 score of 1, with documentation that R24 felt down, depressed, or had little interest or pleasure for 2-6 days. On 2/2/26, a progress note documented that R24 stated, "A doctor that will give me a gun so I can shoot myself in the head." The record review found no documentation that a suicidal evaluation was completed, no documentation that the physician was notified, and no focused care plan problem addressing suicidal ideation. The Social Services Coordinator stated that care plan focused problems were their responsibility, but also acknowledged not knowing why no care plans were in place and that an assessment had not been completed for the suicidal statement. R30 was admitted with diagnoses including chronic kidney disease, type 2 diabetes mellitus, PTSD, bipolar disorder, and schizophrenia. R30’s MDS dated 4/2/26 showed a BIMS score of 15 and no depressive symptoms or behaviors. A psychiatric follow-up note dated 10/31/25 documented past psychiatric hospitalizations and significant trauma, including PTSD related to extensive trauma. R30 told the surveyor that R30 had requested to see a therapist, but the facility did not do anything about it. The Social Services Coordinator confirmed that the facility was not providing in-person psychotherapy services, that R30 did not want zoom psychotherapy, and that the resident was never re-approached about arranging psychotherapy in the community or through the community case manager.
