Food Served at Unsafe Temperatures
Summary
The facility did not ensure food was served at safe and palatable temperatures for 2 residents, R72 and R37. The facility’s Record of Food Temperatures policy required hot foods to be held at 135 degrees Fahrenheit or greater, cold foods at or below 41 degrees Fahrenheit, and no food to be served that did not meet Food Code standard temperatures. R72, who had diagnoses including a nondisplaced right humerus fracture, diabetes, and chronic kidney disease, had a BIMS score of 14 out of 15 and told the surveyor that food was not cooked right, did not taste the way it should, and was always cold. R72 also stated that when R72 requested a hot dog instead of the meal on the tray, the kitchen sent the same food and a CNA eventually obtained a hot dog. During observation of lunch tray preparation, Dietary Aide-L measured R72’s tray and found pork fried rice at 130 degrees Fahrenheit, an egg roll at 134 degrees Fahrenheit, and Jell-O at 43 degrees Fahrenheit, which did not meet the required hot or cold holding temperatures. R37, who had diagnoses including fracture around an internal prosthetic left knee joint, fracture of the left femur, and anemia, also had a BIMS score of 14 out of 15 and described meals as ranging from horrid to very good, including a sparse Mother’s Day meal served in Styrofoam cups. When R37’s lunch tray was prepared, the pork fried rice was 125 degrees Fahrenheit, the egg roll was 128 degrees Fahrenheit, and the Jell-O was 48 degrees Fahrenheit; the tray was the last prepared and placed in the cart for delivery, and the food was not reheated or cooled to proper temperature before being placed in the cart. The surveyor observed the dietary aide place the out-of-range food on the tray without contacting the cook, and later observed the tray cart door left open during delivery.
