Summary

The facility failed to ensure that food and drink were palatable, attractive, and served at safe and appetizing temperatures. During record review, resident interviews, and staff interviews, surveyors found that food temperatures were not being taken for all food items before they left the kitchen. The facility was only recording temperatures for items on the main menu and was not recording temperatures for alternate or always available food items before serving them to residents. The Director of Dining Services confirmed these findings during the kitchen walkthrough and stated that temperatures for those items would start being recorded. The facility’s policy stated that all foods shall be cooked, held, and served at safe temperatures, with hot foods above 135 degrees F and cold foods below 41 degrees F, and that temperatures would be taken once cooked, before, during, and after service. Resident Council members stated the bread was always hard. Residents reported that food was cold at times, breadsticks were hard as a rock, hot food was cold at times, and the food was not good. One meal tray was tested throughout the survey process, and the failed practice was identified as affecting one of one meal tray tested, with the potential to affect more than a limited number of residents.
