Review of the MAR showed the ordered skin cream for a skin infection was not given on multiple days, and the ordered muscle relaxer was not given on two days. The inhaler ordered through a breathing treatment device was not given on three days, with documentation showing staff were waiting for pharmacy delivery and no documentation that the provider was notified. Staff interviews confirmed the medications were not given as ordered, that documentation was missing, and that nurses should have documented why medications were not provided and notified the provider.
