Hand Hygiene and EBP Not Followed During Resident Care

Summary

The facility failed to ensure hand hygiene, glove use practices, and Enhanced Barrier Precautions (EBP) were followed for 5 of 6 staff reviewed for infection control. The facility’s hand hygiene policy stated that hand hygiene is required before and after direct resident contact, after contact with body fluids or contaminated surfaces, before and after wearing gloves, and immediately after removing gloves. The CDC guidance reviewed by surveyors stated that EBP includes targeted gown and glove use during high-contact resident care activities such as changing briefs, assisting with toileting, and urinary catheter care. Resident 9 had physician orders for EBP related to an indwelling urinary catheter and bilateral lower leg wounds. EBP signage was observed outside the resident’s room. During observation, Staff G entered the room wearing a gown and gloves and performed multiple tasks, including cleaning a kidney basin with oral secretions, touching the resident’s belongings, performing peri-care and catheter care, cleaning a bowel movement, and assisting with a gown change, but did not perform hand hygiene between glove changes. Staff H entered the room wearing a gown and gloves, performed peri-care, assisted the resident to roll, held the resident’s hand, applied barrier cream, and touched bedding, a bedside table, and the resident’s gown without changing gloves after peri-care or performing hand hygiene. Staff J entered the room wearing a gown and two pairs of gloves, performed peri-care and bowel movement care, touched the bed controls, sheets, pillow, and trash bag, then emptied the catheter bag and moved the bedside table, but did not perform hand hygiene between glove use. Staff K and Staff L entered the room and applied gloves; Staff K performed care to the resident’s back and bottom without hand hygiene between glove use, and Staff L cleaned the resident’s bowel movement and then performed peri-care and catheter care with the same gloves. Staff K stated the resident was on EBP due to the catheter, and Staff L stated they should have followed the EBP signage and worn a gown while performing care. The DON stated staff were expected to follow the hand hygiene policy and EBP signage.
