Summary

The facility failed to ensure sufficient nursing staff were available to provide assistance in a timely manner and to have a licensed nurse in charge on each shift, as reflected in grievance records, staff interviews, resident interviews, and staffing review for Units 1 and 3. The facility policy stated staffing would be sufficient to assure resident safety, maintain highest practicable well-being, and meet resident needs, with state minimums not necessarily determining sufficiency. On 05/18/2026, the State Agency received information that management was aware of staffing concerns but had not made changes in staffing. The grievance log for 05/16/2026 through 06/15/2026 showed four grievances about call light response time and one grievance about missed showers, with four of the five grievances occurring on Unit 1. One resident reported longer-than-usual call light times, and the audit showed waits up to 23 minutes; another reported long waits, with audit times up to 35 minutes. A resident on Unit 3 reported being told staff had 20 minutes to answer the call light, and the audit showed waits up to 34 minutes, including an emergency response time of 13 minutes. Another resident reported waiting over an hour for a call light to be answered, and a resident reported missing a scheduled Monday shower. Staff and resident interviews described ongoing staffing concerns, especially on Unit 1, where there were more heavy care residents requiring mechanical lifts and assistance with activities of daily living. The census showed 37 residents on Unit 1, and information from staff indicated 19 residents required two-person mechanical assistance. On the day reviewed, Unit 1 had three NAs assigned for day shift. Staff reported they usually had 10 to 13 residents each, did not always have time to complete all assigned tasks, and sometimes could not take lunch breaks. A resident stated call lights were usually answered in about 20 minutes or longer, while another said the unit needed three staff instead of two. The RN resident care manager and the DON both stated they would not expect residents to wait 20 minutes or longer for call light response, and the administrator acknowledged staff had reported needing more help on the floor.
