Summary

Failure to provide necessary and adequate supervision for a resident at risk for elopement allowed the resident to exit the facility unnoticed. The resident had diagnoses including dementia and secondary parkinsonism, and the admission MDS showed severe cognitive impairment. The elopement evaluation described the resident as disoriented all the time, agitated, ambulatory, independent with mobility, taking medications for verbal, physical, or emotional outbursts, and wandering through the facility, wandering into other rooms, and exit seeking. The care plan identified goals that the resident would not leave the center unattended and would not elope, with interventions including a wander guard bracelet and diversionary activities. On the day of the incident, the facility document titled 15 Minute Check showed the resident was last seen in the dining room at 2:15 PM, but there was no documentation of 15-minute checks between 2:30 PM and 9:45 PM. A police incident report showed police received a report at 4:04 PM that the resident was walking and disoriented, and police were with the resident at 4:05 PM near a dealership at 212th SW and 99 interstate, approximately 0.4 miles from the facility and about an eight-minute walk away. The facility investigation report also stated the resident was last seen in the facility at 4:05 PM. Interviews confirmed staff knew the resident was at risk for elopement and should not have left the facility unattended. A CNA stated the resident was not in the room around 2:40 PM, was not in the dining room, and walked independently while refusing to use a walker. The Charge Nurse stated staff were expected to search the facility, call 911, and notify family, the doctor, law enforcement, the DON, and the Administrator when a resident was missing, and acknowledged the resident should not have left unattended. The DON and Administrator also stated the resident should not have left the facility unattended.
