Summary

The facility failed to ensure that one resident was adequately monitored and supervised while walking to the ER. The resident had a history that included a right cranioplasty and a prior fall in January 2026 that resulted in an acute subdural hematoma requiring a craniectomy. On 05/25/2026, the resident complained of shortness of breath, chest pain, and left arm pain; the LPN offered to call 911, but the resident refused and preferred to walk to the ER. The progress note did not indicate that a staff member accompanied the resident or that the ER was contacted ahead of time. Similar documentation was found on 05/28/2026, when the resident reported anxiety and wanting medication and walked to the ER, and on 05/30/2026, when the resident complained that something was wrong with their head and walked to the ER, again without documentation that staff accompanied the resident or notified the ER. The resident’s care plan and Kardex documented that the resident could walk in the room and halls with distant supervision using a front-wheeled walker, but they did not address the resident’s ability to walk outside of the facility. In interviews, the resident stated that when they told staff they wanted to go to the ER, they were scolded by nurses and therefore walked there on their own. Staff interviews reflected that residents should not walk to the ER on their own; one LPN stated they would normally assess the resident and call 911, another stated they would not allow a resident to walk to the ER for safety reasons, and a resident care manager stated that if a resident insisted on walking, staff would educate the resident, call the ER, follow up to confirm arrival, and document the event. The Chief Nursing Officer also stated that a resident should not be walking on their own to the ER.
