Failure to Maintain Heating and Laundry Equipment

Harrisonburg Hlth & Rehab CntrHarrisonburg, Virginia Survey Completed on 02-13-2026

Summary

The facility failed to ensure a functioning heating system for one resident in room [ROOM NUMBER]. During an interview, the resident stated the permanent heating system had not been working for months and that a portable electric heater was being used to heat the room. The maintenance director later explained that the facility’s heat came from a boiler system, that a coil had busted in the resident’s room causing water to escape, and that the heating unit had been taken off-line. The maintenance director stated the resident refused to move out of the room, so a portable heater was placed there, and estimated the heating system may have been down since October 2025. No work order or repair quote documentation was available at the time of the interview. The facility also failed to maintain essential laundry equipment. During a tour of the laundry department, large amounts of mixed soiled facility and resident personal laundry were observed in bags on the floor at the bottom of the laundry chute, along with multiple rolling bins of soiled laundry waiting to be washed and folded, piles of clean facility linen, and clean resident clothing ready to be returned to the units. Only two of four washers and two of four dryers were observed operating. The EVS Director stated that three of four washers and three of four dryers were operable, but also said the washer had recently been repaired and then broke again. The maintenance director stated one broken washer had been repaired but needed a part that had not been obtained, and said the malfunctioning dryer worked but took longer to heat and was used last. No work order documentation for the washer was provided.
