QAPI Failure to Sustain Cleanliness and Linen Availability
Summary
The facility failed to maintain an effective QAPI program to correct previously identified concerns related to a clean, comfortable, homelike environment and the availability of sufficient linen for resident care. Surveyors found the facility unclean across all three resident units, with severely soiled floors, overcrowded rooms, and inadequate linen supplies. Linen carts contained only a few pillowcases and about two towels, with no washcloths or blankets available, and several resident rooms lacked facility-provided bedspreads, leaving some residents using personal blankets from home. Multiple resident rooms were observed with boxes, food items, personal belongings, and plastic storage containers placed in front of and on top of air conditioning/heating units, restricting airflow. Rooms contained soiled linens, balled-up clothing, spilled sauces and spices on overbed tables and bedside cabinets, stained privacy curtains, spoiled food with visible blue mold, and mixed food and drink items that were unidentifiable. One resident was observed lying on a soiled bed encrusted with spilled food, wearing a sweatshirt stained with dried food and liquids, and was naked from the waist down and partially covered by a soiled sheet. The roommate’s side of the semi-private room was described as similar. Resident and staff interviews confirmed ongoing problems with housekeeping, laundry, and missing personal items. A resident reported his room had not been cleaned for approximately two weeks and that there was not enough linen for his bed or a bedspread. A housekeeper stated she was the only housekeeper working that day, that daily room cleaning did not occur, and that rooms were often cleaned only every three to four days or once a week. Laundry observations showed excessive piles of soiled clothing and linens stacked in the laundry room, overflowing bins, wet laundry piled in front of the doors, and washers and dryers full of items sitting unattended. An LPN reported the facility was short on linen, lacked enough laundry staff, and that laundry left too long became molded and was thrown away. Grievances and Resident Council minutes also reflected repeated concerns about rooms not being cleaned and personal items missing and not returned.
