Homelike Environment Not Maintained in Resident Rooms, Bathrooms, and Shower Area
Summary
The facility failed to maintain a safe, clean, comfortable, and homelike environment for multiple residents and in several common areas. Resident #227, whose most recent BIMS score was 12 out of 15 indicating moderate cognitive impairment, had a large black scuff mark and multiple discolored yellowish painted areas on the wall in the room. The resident stated the scuff and wall discoloration had been reported many times and had remained unaddressed for a long time. Staff interviews confirmed there were no active work orders for the room, and the maintenance director and administrator stated the walls were old, the building was being repainted, and the scuff would be covered when painted. Resident #228, whose BIMS score was 15 out of 15, had a bathroom door that would not fully close, a bathroom baseboard separating from the wall and held with push pins, and small round holes in the wall behind the toilet from removed grab bars. The resident stated the baseboard had been coming off, the door was warped, and the holes remained after the grab bars were removed. The maintenance director acknowledged the door needed to be trimmed to close properly and that the holes behind the toilet needed to be filled and painted. The administrator observed the room and stated the condition looked like a repair that would be done at home. Resident #237, whose BIMS score was 15 out of 15, had a bathroom with significant moisture damage beside the toilet, bubbled and uneven wall surfaces, and blackened debris buildup between floor tiles. The resident described the bathroom as nasty and said staff had told him it would be fixed, but no one had come to do it. Housekeeping stated bathrooms were cleaned daily, but the director of housekeeping observed the floor needed cleaning and said it could not be completely cleaned because it needed replacement. The maintenance director stated the bathroom tiles should have been replaced and that the wall needed repair, while the administrator noted the bathroom floor had not yet been replaced. Additional areas were also found not maintained in a clean or homelike condition. The South hallway shower room had black slime-like buildup between the white tiles on the lower portions of the shower walls, and the director of housekeeping stated the buildup was an ongoing problem due to moisture in the room. Two room bathrooms contained brown splatter or brown material on the toilet riser, toilet bowl, toilet rim, wall above the trash can, and floor, and the director of housekeeping stated one of the bathrooms needed to be cleaned. Two room clocks were also observed to be inaccurate, with the administrator acknowledging that one clock was an hour behind and another was not set to the current time.
