Summary

The facility failed to ensure that a resident with limited ROM received the ordered splinting treatment to support right hand mobility. Resident #4 was admitted with diagnoses including hemiplegia and hemiparesis following cerebral infarction, right hand contracture, and cognitive communication deficit. The physician ordered a right hand splint to be donned in the AM and removed in the PM or as tolerated, with hand hygiene before donning and monitoring for skin irritation, breakdown, swelling, and pain. The resident’s care plan included encouragement, supervision, and assistance with splint use as recommended, and the resident was documented as having functional limitations in upper extremity ROM and dependence for multiple activities of daily living. On the morning of the observed event, Resident #4 was sent to a medical appointment without the splint on her right hand. Later that day, the resident was observed sitting in a wheelchair with a rolled towel in her right hand instead of the splint. The family member reported the resident was not wearing the splint at the appointment and said they had been told rehabilitation did not know when the splint should be worn. The Director of Rehabilitation stated nursing staff were responsible for putting on and removing the splint per the order, while the DON stated the CNAs put the splint on but nurses were responsible for ensuring it was done. A CNA said she did not put the splint on after getting the resident up because it was a busy morning, and a nurse said she did not put it on because she did not believe the order was still active.
